Kerry Taylor and members of his unit were at Landiing Zone Sally when, at 0850 on 21 January 1970, a box of 81 mm mortar ammunition exploded due to unknown causes while being moved by troops of the 199th Light Infantry Brigade. This explosion resulted in the deaths of the following members of 4th Bn/12th Inf:
