I believe this is the death certificate of John Thomas Stillwell, born 3 Nov 1862 in Hannibal, MO. The death dates match to a headstone in Oakhill Cemetery, Whitewright, Fannin County, Texas. I don't have an explanation as to why the first initial on the death certificate is "W" instead of "J".commented on this image. (10 Dec 2010)
