Corporal Jack R. Chambers was killed on Sept. 13 near the Albert canal in the town of Hasselt Belgium. He was attached to the 67th Armored Regiment, 2th Armoured Divsion recon unit. On a patrol near the canel his Captain Harold D. Smith as well as Pvt Clyde W. Madison where also killed by enemy fire. A memorial honoring them stands in the town square of Hasselt as well as another memorial near the location where they where killed.
