On this 26 day of December, one thousand eight hundred and eighty seven, personally appeared before me, John H. Russe, Clerk of the Circuit Court, a Court of Record within and for the County and State aforesaid, John McKee, aged 44 years, a resident of Lawrenceburg in the County of Dearborn in the State of Indiana, who, being duly sworn according to law, declares that he is the identical John McKee who enlisted in the service of the United States at Terre Haute, County of Vigo, State of Indiana on the 1 day of November 1861 as a Private in Company H of the 2d Regiment of Indiana Cav., commanded by Capt. J. B. Presby and was honorably discharged at Edgefield, state of Tennessee on the 22 day of July 1865. That his personal description is as follows: Age 44 years; height 5 feet 10 inches; complexion Dark; hair Black; eyes Black. That while a member of the organization aforesaid in the service and in the line of his duty, at or near a place called Pittsburgh Landing at Tennessee on or about the month of April 1862.
Contracted Malarial Poisoning resulting chronic diarrhea caused by malaria, exposure impure water and improper cooked diet.
That he has not been employed in the military or naval service otherwise than as stated above except Veteran Co. B 2d Regt. Ind. Cav reorganized. Since leaving the service he has resided in Dearborn County Ind. and his occupation has been that of a Cooper. That prior to his entry into the service above named, he was a man of good, sound, physical health, being when enrolled a Cooper. That he is now physically disabled from obtaining his subsistence by manual labor by reason of his injuries above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placeon the invalid pension roll of the United States.
He hereby appoints with full power of substitution and revocation, Maning & Slusher of Columbus, State of Indiana, his true and lawful attorney, to prosecute his claim. That he has not received nor applied for a pension. His postoffice address is Lawrenceburg, County of Dearborn, State of Indiana.

Sometimes he is wholly disabled and some time partially and even when at labor can not do near the labor of a sound man, and think my estimate above a fair one during each and every year.

He makes the following statement upon which he bases his claim for:

Never had fever and ague in army nor fever of any kind. Had diarrhoea in army and ever since at times averaging every two weeks lasting from a few days to 2 weeks, then very easier for a few days when again suffers from diarrhoea four to eight discharges daily, first large then small ?
Upon examination we find the following objective conditions: Abdomen normal size - tympanitic generally - tender skin tawny? - conjuctiva muddy - Liver normal size but tender - spleen normal - muscles fair - in fair flesh - tongue fumeal fissured.
Heart lungs normal. Traces of sternal piles, sphincter relaxed - mucous membrane congested - relaxed - vessels enlarged - 2 piles on anterior surface - one on each lateral and one on posterior surface - all rather larger than a navy bean. No ulcers, no abrasions - no bleeding. Lungs normal - no signs of malarial poisoning - Has appearance of stout healthy man.
He is in our opinion entitle to a 0 rating for the disability caused by malarial poisoning, 8/18 for that vaused by diarrhoea and result disease of rectum.

And in 1862 he came home on a sick furlow - We thought for some time that he would die with the chronic diarrhoea - but he finally recovered and went back to his Regt. after about one year. He was discharged about July 28 1865 and I have seen him nearly every day sence that time and know that he has constantly suffered from chronic diarrhoea and is at least disabled from performing manuel labor half time. I know he has been doctoring every sence he was discharged from the army.

On this 1 day of July AD 1890, personally appeared before me a Clerk Circuit Court in and for the aforesaid County duly authorized to administer oaths John McKee, aged 47 years, a resident of Lawrenceburg in the County of Dearborn, State of Indiana whose Post Office address is Lawrenceburg Indiana and who, being duly sworn declares as follows that:
he is the identical John McKee who was enrolled on November 1 1861 in Co. H 41st Regiment Indiana Volunteers (2d Cav) and was transferred to Co. B same Regiment January 1 1864 and honorably discharged therefrom on the 22d day of July 1865 at Edgefield Tennessee. That he is a pensioner of the United States Certificate No. 455964 at the rate of $6.00 per month for chronic diarrhea and resulting piles and has pending a claim for increase pension.
He further says he is a sufferer from an ulcerated left leg which disease is of a permanent character and not a result of his own vicious habits and he believes he is disabled in a total pensionable degree thereby.
He makes this declaration for the purpose of securing a rating of pension in accordance with the provisions in the Act of Congress approved June 27 1890.

Got diarrhoea in '62. Rheumatism since '62. Diarrhoea: recurrent involving stomach, liver, rectum with chr. ulcer in left leg. Had chills in '62 - '63. Is short of breath - has smothering spells, cannot then sleep.
Upon examination we find the following objective conditions: Well muscled and nourished. Skin rather florid. Lungs normal. Heart - ? hyper trophy on ? - apex ? and to left nipple ? one ? ?n shows mitral insufficiency. Liver slight enlargement and tender. Stomach distended, tender. Spleen not enlarged. Tongue slightly coated. Crepitas in right shoulder. Bowels - two medium pile tumors internally well congested. Vessels gorged, no loose accumulation of fecal matter. No special evidence of ural poisoning.
He is in our opinion entitled to a 4/18 rating for the disability caused by chr. diarrhoea, 4/18 for that caused by dis. of rectum resulting and 2/18 caused by rheumatism, 4/18 resulting heart dis. 0/18 liver, 0/18 stomach, 0/18 malaise.

…That he (John McKee) believes himself to be entitled to an increase of pension on account of too low a rating for and an increased disability from chronic diarrhea and resulting piles. He further says he is now afflicted with rheumatism and locomotor ataxia which he claims to be a result of said chronic diarrhea. He asks that he be sent before the board of examining surgeons at Osgood Indiana for examination.

GENERAL AFFADAVIT
Personally came before me (blank) in and for said County and State Thomas E. Craig, aged 54 years, citizen of the town of Lawrenceburgh, County of Dearborn, State of Indiana
I have been acquainted with John McKee formerly of Co H 2nd Regt. Indiana Cavalry since his return from the army in 1865. I was then practicing medicine at Manchester in this County, at which place his father lived and where he made his home for a time after his discharge.
I treated him for muscular rheumatism involving the muscles of the back during parts of 1866 and 1867. He left Manchester shortly after that time and I have not treated him until during the past five years since I have resided in this place. I find he has been a constant sufferer from rheumatism, never being free from it and for which he receives a small pension. But I found another train of symptoms had developed as a result in all probability of the rheumatic affliction. I noticed his unsteady gait resembling a drunken man and that in walking the heel was first brought down and then the foot in the usual manner of ?. Examination showed an entire absence of the reflexes except a slight ? reflex in right leg. He cannot stand if his eyes are closed but would fall forward on his face if not prevented. He has great difficulty in walking on account of stumbling but walking after night is almost impossible, so too is ascending and particularly descending stairs. The pupils of his eyes do not respond to the stimulus of light but remain nearly stationary.
He complained of a bandlike pain around the waist and suffers greatly from shooting pains in his legs as well as from cramps in the muscles of the legs and feet.
He has the sensation of cushions under his feet when walking and has a disagreeable tingling sensation in his feet and the same sensation has invaded his left hand and arm during the past few months.
Vision is greatly impaired and on closing his eyes he sees flashes of bright colors. He is gradually losing the use of his limbs. The change in that direction during the past year being quite rapid comparatively. He is totally disabled and cannot perform any kind of manual labor; in fact, he requires the aid of some person to dress himself.
The disease is progressive and I have no hopes of staying its course. I have no interest in the prosecution of this claim.

We hereby certify that in compliance with the requirements of the law, we have carefully examined this applicant who states that he is suffering from the following disability incurred in the service, viz:
Chr. Diar. and piles, rheumatism, Locomotive Ataxia. Ulcerated left leg. Malarial poisoning and results
and that he receives a pension of Six dollars per month.
He makes the following statement upon which he bases his claim for Increase.
My left leg hurts me and gets so weak I can scarcely walk on it. I have Diarrhoea and constipation alternatively. I have smothering sensation at times and heart palpitates. At times I have chills and fever, occasionaly stomach hurts after I eat have sour stomach, have dull aching pains in lower part of body and troubled with cramps and sharp pain in legs. Have dull ache through head, not much dizziness. I fall at times, worse in the dark, have some difficulty in starting water. Have sense of cushion beneath feet. Have noises in ears. I see double. Have nausea.   Sense of constriction around abdomen. Pain in head, neck and shoulders. Never had any private disease.
Upon examination we find the following objective conditions:
Pulse rate 96; respiration 28; temperature 97; height 5 fett 10-1/2 inches; weight 160 pounds; age 48 years.
Tongue red and rough. Skin sallow. Stomach and bowels distended and tympanitive and very tender over the stomach. Liver dullness decidedly increased and liver very tender. Spleen enlarged. Specular examination reveals 4 internal piles from 1/2 to 1/3 in. in diameter and bowels inflamed. Anus has a blue appearance.
Rate recommended Six Dollars.
Crepitation in shoulders and knees. Motion in left shoulder and hip somewhat limited. No swelling or contraction.
Rate recommended Two Dollars.
There is two pigmented spots on inner side of left leg size of a half dollar and 2 or 3 size of a quarter and quite a number of small ones and also a varicose condition of internal long saphvenous vein of left leg from knee down to sole of foot internal aspect.
Rate recommended Four Dollars.
Heart. Area of dullness increased. Action frequent. Billious murmur accompanies 1st sound heart at base and apex. Pulse standing 100. After exercise 112.
Rate recommended Four dollars.
Incoordination of movements in walking are exaggerated in the dar. Pupils are alike and respond to light. Tongue protrudes in a strait line. Sensation impaired in four extremeties. Entire absence of knee jerk in left leg and almost so nil in right leg. Hs arcus senilis in both eyes. No evidence of syphyilis.
Rate recommended Four dollars.
He is in our opinion entitled to a 6/18 rating for the disability caused by Chr. Diar. and res. piles, 2/18 for that caused by Rheumatism and 4/18 for that caused by dis. of heart, 4/18 ulcerated leg, 4/18 locomotor atax.

Affiant has known and worked with the soldier for about fourteen years and knows from seeing and being with him that he (soldier) is and has been for the last ten or twelve years continuously afflicted with an ulcerated left leg which was not and is not due to vicious habits, said soldier not being addicted to vicious habits since affiant knew him.
That he had said ulcerated left leg on July 5/90 and Mar 9/92.
Affiant did not write this affidavit, the same was written in his presence and from his oral statements by C. F. Hayes in Jany 1 1894.

Russe, a Notary Public within and for said county and state personally appeared John Williamson, age 24 years, a resident of Lawrenceburg, county of Dearborn and state of Indiana, and whose postoffice address is Lawrenceburg Indiana whom I certify to be reputable and entitled to credit, and who being by me first duly sworn on oath says:
Affiant has known the soldier John McKee all his (affiant's) life and has worked with him in the same shop for about ten years.
Affiant knows from seeing and being with the soldier that he is afflicted with an ulcerated left leg and that said ulcerated left leg is not due to vicious haibts as the soldier is not and never has been during affiant's acquaintance with him addicted to vicious habits.
Solder was afflicted with said ulcerated left leg on July 5th 1890 and from then continuously to the present time including March 9 1892 as affiant well knows. This affidavit was written by C. F. Hayes in the presence of affiant from oral statements made by him on January 1st 1894.

Personally came before me a Clerk Circuit Court in and for the aforesaid County and State, T. E. Craig, M.D., whose Post Office is Lawrenceburgh, Dearborn Co., Indiana, well known to me to be reputable and entitled to credit and who being duly sworn, declares in relation to the aforesaid case as follows:
That he is a practicing physician and has been acquainted with the above-named soldier for about 28 years and that:
I have been his physician for the principal part of the past sixteen years and his constant physician for the past eight years.
Claimant, during my services, has suffered greatly from rheumatism of the joints and of the heart which has resulted in mitrol stenosis and rsulting dilation of the left ventricle of the heart. He has also suffered from chronic varicose veins of left leg. At times the ulcers improve but a relapse follows hard labor. But his most serious disability is locomotive ataxia. The patella reflex and clou? are absent and his walk is very uncertain. He is unable to walk at night when he cannot see where to place his feet. He also suffers from band pain around his chest and lightning pains in the limbs. His sight is also becoming defective.
Notwithstanding the best approved treatment this disability does not leave but rather becomes worse from year to year.
I regard him as totally disabled and unable to perform manual labor.
The above statement is in my own hand and I have no interest in prosecuting this claim.

Little, a Notary Public, within and for said county and state personally appeared Aaron Keller age 50 years, a resident of Connersville, county of Fayette and state of Indiana and whose postoffice address is Connersville, Indiana whom I certify to be reputable and entitled to credit and who being by me first duly sworn on oath says:
I have known said claimant since 1866 at Lawrenceburg Indiana. During five years from 1866, I saw him every week and since 1871 I have seen him frequently. While at Lawrenceburg fronm 1866 to 1871, he was complaining of chronic diarrhoea and rheumatism. He walked lame and seemed to be in poor health then and claimed that the above named diseases were what ailed him and claimed that he contracted them in the army. Since 1871 when I would see him he still complained of suffering from said diseases and used a cane in walking. Have had him leave me to obey natures call from diarrhoea. I have not see him since September 1893 and he was still suffering from said diseases at that date as in 1866.
The foregoing statement was written by said Notary on January 9 1894 at his office in Connersville Indiana at my oral dictation in my presence and in making said statement I was not aided by any person or by any paper or statement whatever not attached as an exhibit hereto.

He is the above named claimant that he is afflicted with the rheumatism, disease of heart and locomotive ataxia. That said diseases are permanent in character as he believes and are not due to vicious habits and totally disables him from earning a support by manuel labor.
That he filed a prior application on July 5 1890 and he makes this as a supplemental or additional application to his said prior application for an disabled pension under the Act of June 27 1890 and he hereby appoints Charles F. Hayes of Lawrenceburg Indiana as his Attorney in his said claim.
As evidence in his claim under Act of July 14/62 he says he is unable to furnish medical evidence as to medical treatment received since his discharge from the army until now other than the evidence of Dr. Craig filed herewith for the reason that the other physicians "viz" Drs. Terrill and Harding are now dead. This affidavit was written by D. F. hayes in affiant's presence and was dictated by him with neither assistance from any other person or paper not hereto atached.

He is a brother of the above named soldier John McKee and was at home at his father's residence on a farm in Dearborn County Indiana in July 1865 when said soldier returned home to the same place after being discharged from the U.S. Military service. Seen him as soon as he got home, since then we have been together. When he came home as above stated he was sick and afflicted with chronic diarrhoea and rheumatism to such an extent as to be unable to labor more than half of his time. He has ever since he came home been sorely afflicted with said diseases which have progressed and gradually grown worse until for the last later years he has been fully disabled for the performance of manuel labor. He was lame, very thin in flesh and weak, and complained to me of said diarrhoea and rheumatism. I knew that he was treated by Dr. Terrill of Lawrenceburg Indiana for said chronic diarrhoea and rheumatism and that Dr. Terrill is now dead.
The foregoing statement was written by C. F. Hayes at his office in Lawrenceburg Indiana on Jany 13/94 at affiant's oral dictation in affiant's presence and he making said statement affiant was not aided by any person or thing or any papers which is not attached herewith.

Declaration for Increase of An Invalid Pension
Old and New Disability
…He (John McKee) further declares that he is disabled in the following manner, to wit: chronic diarrhea and resulting piles and rheumatism.
Application is also hereby made for increase of pension on account of a new disability, to wit: at Pittsburgh Landing in the State of Tennessee on or about the 6 and 7 of April 1862, he, by reason of being exposed to the inclement weather without protection and with having to sleep on the wet ground without protection contracted rheumatism and was sent to a field hospital near Corinth Miss. where he was treated for same.
He was then sent to hospital at Hamsburg Landing, Tennessee and from there to Terre Haute Indiana. He asks to be sent to the Osgood examining surgeon at Mooresburg? Ind for examination under this application and that he hereby appoints with full power of substitution and revocation Hayes & Diment of n.w.cor. 5th & Race St., Cincinnati, Ohio, his true and lawful attorneys to prosecute his claim.

Proof of Incurrence of Disability
State of Indiana, County of Dearborn, ss:
Personally appeared before me, a Notary Public in and for the aforesaid County and State, duly authorized to administer oaths, Richard O'Moore aged 53 years, a resident of Lawrenceburg in the County of Dearborn and State of Indiana who, being duly sworn according to law, states that he is acquainted with John McKee, applicant for Invalid Pension, and knows the said John McKee to be the identical person of that name who served as a priv in Company H 41 Regiment of Indiana and who was discharged at Nashville Ten on or about the 28 day of July 1865 by reason of expiration of term of services.
That the said John McKee, while in the line of his duty, at or near Pittsburgh Landing in the State of Tenn. died on or about the 6 and 7 day of April 1862 become disabled in the following manner, viz: by reason of being exposed to the inclement weather and having to sleep on the wet ground without suitable protection said soldier John McKee contracted rheumatism which continued to afflict him during the time he remained together with the service.
That the facts as above stated are personally known to affiant by reason of my being at Pittsburgh Landing and seeing him sick and afflicted there with rheumatism and knowing that he was sent to the hospital at Corinth Miss. on account of same.
This affidavit was written by C. F. Hayes on Feby 15 1896 in affiant's presence and from his oral statements and he was not aided by any other person or by any written recital not hereto attached.
